Name mismatch or name change: the affidavit and what SSC asks for

Quick answer

If your name changed after Class 10, SSC asks for proof of the change at document verification. After marriage that can be your husband's passport showing both names, an attested marriage certificate, or a joint affidavit from husband and wife with a joint photograph sworn before an Oath Commissioner. For any other change, men and women alike, SSC asks for three things: a Deed Poll or affidavit sworn before an Oath Commissioner, original cuttings from two leading daily newspapers (one from your area), and the Gazette notification.

If it is only a spelling difference, SSC tells you to enter your name exactly as it appears on your Class 10 certificate. Start there. If someone asks you for an affidavit anyway, the form below writes one.

(SSC CGL 2026 notice para 15.11.9 and Annexure-III; SSC CHSL 2026 notice para 16.5.9; checked 2 November 2026.)

Our draft. SSC prints the documents it wants but no affidavit format, so the wording below is ours. Pick your case, fill in the form, and the affidavit appears in English, Hindi or Bengali.

This is a format, not legal advice; the notary or issuing office has the final word.

What SSC asks for at document verification

15.11.9 A candidate who claims change in name after matriculation on marriage or remarriage or divorce, etc. the following documents shall be submitted:
15.11.9.1 In case of marriage of women: Photocopy of Husband's passport showing names of spouses or an attested copy of marriage certificate issued by the Registrar of Marriage or an Affidavit from husband and wife along with a joint photograph duly sworn before the Oath Commissioner;

SSC accepts any one of three: this affidavit, your husband's passport showing both names, or an attested marriage certificate from the Registrar of Marriage. If you have the passport or the certificate, you may not need this affidavit.

When SSC checks this

  • At the form stage SSC asks for your name, father's name and mother's name "exactly as given in the Matriculation (10th Class) Certificate". If your name changed after Class 10, the form has a place for it: "indicate the same at Sl. No. 3a and 3b". (CGL 2026, Annexure-III, Part-A item 6, p. 60.)
  • The user departments run document verification after the final result (para 15.1), and the change-of-name papers above are on SSC's DV list (para 15.11.9). The DV admission certificate can ask for more ("Any other document specified in the Admission Certificate for DV", para 15.12), so read it when it comes.
  • On exam day the date of birth has to match. If the date on your admission certificate and on the photo ID or certificate you bring don't match, "the candidate will not be allowed to appear in the examination" (para 14.8, p. 38). That rule is about date of birth, not name, but check it while you sort out the name.
  • SSC's Aadhaar policy says e-KYC can take your name, father's name, gender, date of birth, address and photo from Aadhaar, with your consent. Candidates who don't use Aadhaar upload a supporting document for each of those instead. The policy doesn't say what happens when the Aadhaar name is spelt differently from the Class 10 name, so go by the Class 10 rule above.

Documents to keep ready

Your caseWhat SSC lists (para 15.11.9)What this page gives you
Changed after marriageOne of: husband's passport photocopy showing both names / attested marriage certificate from the Registrar of Marriage / joint affidavit + joint photograph, sworn before an Oath CommissionerJoint affidavit draft
Changed after remarriageDivorce deed or death certificate of the first spouse, AND one of the three aboveJoint affidavit draft
Changed after divorceCertified copy of the divorce decree AND a deed poll or affidavit sworn before an Oath CommissionerAffidavit draft
Changed for another reasonDeed poll or affidavit sworn before an Oath Commissioner AND two leading daily newspaper cuttings in original (one local) AND the Gazette notificationAffidavit draft + checklist
Same name, spelt differently on different documentsNot covered by SSC's notice"One and the same person" draft; fill the SSC form as on your Class 10 certificate

Steps

  1. Pick your case and fill in the form. Check every name against the document it comes from, letter by letter.
  2. Print the draft on stamp paper of the value your state requires. The notary or oath commissioner will tell you the value.
  3. Sign it in front of the oath commissioner (SSC's word) or notary, who completes the verification.
  4. For a change for any other reason, also keep the two newspaper cuttings in original and the Gazette notification. SSC asks for all three together.
  5. Make photocopies. SSC asks for copies at DV; keep the originals with you.
  6. Read your DV admission certificate when it arrives. It can ask for more (para 15.12).

Tips

  • Married and have your husband's passport or a registered marriage certificate? SSC accepts either instead of the joint affidavit (15.11.9.1). The affidavit is only one of the three options.
  • For a change for any other reason, the affidavit alone is not enough for SSC. The newspaper cuttings and the Gazette notification are in the same line of the notice.
  • Fill in the SSC form from your Class 10 certificate, not from Aadhaar. That is the notice's instruction (Annexure-III, item 6).

Frequently asked questions

Do I need an affidavit if my name is different in Aadhaar and my marksheet for SSC?
SSC's notice does not list an affidavit for a spelling difference. It asks you to fill in your name exactly as on your Class 10 certificate (CGL 2026, Annexure-III). Keep a "one and the same person" affidavit ready only if an office asks you for one.
What documents does SSC ask for a name change after marriage?
Any one of three: a photocopy of your husband's passport showing both names, an attested marriage certificate from the Registrar of Marriage, or an affidavit from husband and wife with a joint photograph, sworn before an Oath Commissioner (para 15.11.9.1).
Is a Gazette notification needed for a name change for SSC?
For a change of name in "other circumstances" (not marriage, remarriage or divorce), yes. SSC lists a deed poll or affidavit, two newspaper cuttings in original and the Gazette notification (para 15.11.9.4).
What stamp paper value does the affidavit need?
It depends on your state. SSC's notice doesn't say. Ask the notary or oath commissioner who will attest it.
Is this an official SSC format?
No. SSC prints the list of documents but no affidavit format. The text this tool writes is our draft, and the page says so.
Does this page keep what I type?
No. The affidavit is built in your browser. Nothing is sent to us or stored.
